The Chart Swap Department is responsible for processing attorney requests, (LOA’s), for billing records, affidavits, subpoenas, and balance verifications. The department will contact the attorney or legal assistant by comment in Chart Swap if additional information is required.



  • Review all requests for HIPAA compliance prior to processing.
  • Check in the IDX system to verify patient.
  • Process subpoenas and shipment pending daily; other requests will be processed by date received/requested.
  • Perform all other duties and assignments as requested.


Job Requirements


  • High school diploma or equivalent required
  • Six months previous experience working with insurance in an office or hospital environment.
  • Computer proficient
  • Ability to focus and complete assignments with minimal supervision

  • This job will be performed in a well-lighted and well-vented environment.
  • Prolonged sitting tolerance is required.
  • Must be able to lift 10 to 25 pounds


This position may require manual dexterity and/or frequent use of the computer, telephone, 10-key, calculator, office machines (copier, scanner, fax) and/or the ability to perform repetitive motions and/or meet production standards to comply with the essential functions. Also, may require physical and/or mental stamina to work overtime, additional hours beyond a regular schedule and/or more than five days per week.
