Job Description

 • Establishes and maintain rapport with Hospitalists, Health plan’s case managers, hospital’s utilization and case managers support personnel and PCP’s offices as well as Group’s case management department.

• Work closely with Lead Physicians, Hospitalists and company’s Medical Director.

• Assist Hospitalist if requested and as applicable, in obtaining and organizing medical charts of the corresponding patients in the participating Hospital.

• Reviews and process daily census against hospital information obtained.

• Maintains and process face sheets with pertinent information obtained from various external sources for database entry purposes for end of month reconciliation reports.

• Assist with Medical Records retrieval if requested by the Hospitalists and in conjunction with the Hospital’s Medical Record Department.

• Calls Primary Care Physician offices with patient’s discharge information and/or orders as dictated by the Hospitalist.

• Informs Primary Care Physician offices of their patient’s hospital admission.

• Process any new admission, discharges, expirations and transfers daily.

• Coordinates the patient follow up appointment upon discharge with the PCP’s office.

• Assists Hospitalist with arranging outpatient consults when necessary.

• Ensures the integrity of data collection, analysis and reports.

• Creates, maintains, and communicates administrative tools and reports for internal staff, Physicians, providers and health plans.

• Designs reports and presentations for providers, management, and utilization management department.

• Promotes, establishes, and maintains excellent rapport with Provider community in order to increase program’s visibility in the hospital.

• Collects and process monthly physician’s schedule.

• Participates in projects assigned by the Sr. V.P. of Operations.

• Ensures compliance with Company’s established policies and procedures.

• Performs others duties and responsibilities as assigned.

• Willing to work occasional evenings attending physician programs as deemed appropriate

 

Job Requirements

QUALIFICATIONS / EXPERIENCE:

• Experience in health care environment, i.e., hospital, health plan or Physician’s medical office setting.

• 2-year College degree preferred.

• Demonstrates proficiency with database management and software programs, i.e.; Microsoft Word, Excel, PowerPoint, Lotus Notes and any others.

• Excellent verbal and written communication skills required.

• Proficiency in Medical Terminology and familiarity with clinical documentation process

• Must possess superior organizational and interpersonal skills.
