Job Description

This division leadership position provides overall direction and management of the documentation process for the division. By utilizing industry and company best practices, as well as current and trended reports of individual providers and facilities, the Clinician Documentation Education Director (CDE Director) is charged with ensuring that proactive, professional and specific feedback is routinely provided to TeamHealth-affiliated physicians, providers and midlevel practitioners. CDE Directors are to collaborate with providers and division/corporate staff to facilitate a continuous quality improvement culture to hardwire required documentation standards, improve patient care, reduce associated risk management issues and comply with reimbursement guidelines.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Works to develop comprehensive education plans that utilize resources in the most efficient manner for training of providers related to documentation.
  • Ensures that Documentation Specialists are knowledgeable regarding documentation standards, have strong interpersonal skills to collaborate with providers, are able to analyze and act upon mid-month and month-end reports related to individual providers and facilities, and can effectively prioritize their education activities.
  • Offers formal and informal training of Documentation Specialists. Completes performance evaluations and assists in setting goals for Documentation Specialists.
  • Analyzes and monitors providers’ documentation and identifies improvement opportunities as compared to industry and TeamHealth benchmarks.
  • Works with Documentation Specialists and RMDs/FMDs to ensure new providers are offered a thorough orientation ideally within their first week, but definitely no later than 20 days of hire date. “New” providers may be recently recruited physicians or midlevel practitioners who joined a TeamHealth division via a new facility contract or merger/acquisition.
  • Supports and designs processes that allow providers to receive specific, individualized feedback illustrated through examples of their own documentation and how it crosswalks to various medical coding components and legal implications. Organizes documentation efforts in an effective, efficient manner, while considering education of new providers, education of outlier providers, proactive ongoing education and education on targeted documentation initiatives.
  • Conducts provider education via face-to-face meeting, conference calls, Web Conferencing and group facility meetings.
  • Oversees and monitors the provider education offered by Documentation Specialists and supervises their usage of face-to-face meetings, conference calls, Web Conferencing and group facility meetings.
  • Communicates in a timely fashion to providers, Documentation Specialists and other staff regarding any changes to the current documentation requirements and medical coding criteria as dictated by a governmental agency, payer and/or TeamHealth. Analyzes and monitors assigned providers’ documentation and identifies improvement opportunities as compared to industry and TeamHealth benchmarks; utilizes mid-month and month end reports that reflect results of individual providers and specific facilities.
  • Interacts with various hospital personnel, as needed, to facilitate the charting process and allow for billing of the provider's services rendered.
  • Assists RMDs/FMDs with deployment of templates and corporate documentation initiatives (i.e., Progress Note Template, Consultation Request Template, Discharge Sheet Template, etc.) to their facilities accompanied by related education and support of providers.
  • Offers in-services to providers regarding new technologies or methodologies for generating medical charts, which may involve computer training on new software.
  • Interacts with RMDs/FMDs concerning outlier providers with undocumented services and positively strategizing on how to affect these providers’ future documentation.
  • Understands and utilizes the Escalation Policy concerning outlier providers and lack of documentation improvement after appropriate education efforts have been exhausted.
  • Updates the division’s Chief Operating Officer and Client Services Vice Presidents on Undocumented Services Report results and trends via distribution of monthly reports, division-specific Documentation Balanced Scorecard and 30/60-day Action Plans.
  • Partners with TeamHealth’s Provider Documentation Operations Manager regarding the hardwiring of Documentation best practices and standards, actively involved with various documentation initiatives and collaborates to resolve larger Documentation issues.
  • Attends monthly Documentation Manager Update Meetings with Operations Manager.
  • Communicates daily and/or weekly with Documentation Specialists and Documentation Team Coordinator to remain abreast of current efforts, potential problem areas.
  • Assists in communicating policy changes to Documentation Specialists and Documentation Team Coordinator in a timely manner and is available and/or seeks assistance from Operations Manager or Human Resources to answer questions relative to corporate policies and procedures.
  • Oversees work schedules and approves personal time off (PTO) for Documentation Specialists and Documentation Team Coordinator in accordance with TeamHealth policy. Job Requirements

The job qualifications for this position are:

  • Clinical expertise is required preferably in an ED and/or HM setting, nursing experience desired;
  • Quality Improvement/Quality Assurance background very beneficial;
  • Proven CPT-4 and ICD-9 coding experience;
  • CPC certification preferred;
  • Willingness to receive coding training and sit for CPC certification;
  • Previous supervisory/management experience;
  • Strong analytical and statistical skills and experience;
  • Excellent interpersonal skills;
  • Excellent presentation skills;
  • Excellent verbal and written communication skills;
  • Excellent organizational skills;
  • Good computer skills (word-processing, spreadsheet and database management);
  • Ability to support multiple persons, tasks and deadlines;
  • Ability to handle stress and conflict; and

PHYSICAL / ENVIRONMENTAL DEMANDS:

  • Primarily performed within a modern, hospital setting, usually within the ED;
  • Frequent travel (out-of town and overnight; via vehicle and commercial/commuter airplanes);
  • Moderate to heavy stress levels;
  • Occasional lifting, bending, standing, stooping and reaching;
  • Moderate to prolonged sitting; and
  • Moderate to prolonged work at a PC/Computer.

This position may require manual dexterity and/or frequent use of the computer, telephone, 10-key, calculator, office machines (copier, scanner, fax) and/or the ability to perform repetitive motions and/or meet production standards to comply with the essential functions. Also, may require physical and/or mental stamina to work overtime, additional hours beyond a regular schedule and/or more than five days per week.
