Job Description

The Clinical Educator prepares all clinicians to work in the PALTC environment, educates and nurtures a Value Based Practice culture, and assists the leadership team in assessing individual and group productivity/efficiency. The West region consists of Washington (Seattle, Spokane), Denver, San Francisco, Arizona (Phoenix, Tucson).

 

  • Oversee clinical education process for clinicians, including orientation, on-boarding and mentoring, ensuring standardized clinical approach and proper clinical documentation
  • Assist with education on proper documentation for correct coding in conjunction with the Clinician Documentation Educator (CDE)
  • Assist with gHERiMED training and troubleshoot clinician efficiency with gHERiMed.
  • Assume responsibility for ensuring that clinicians have an adequate knowledge base and understanding of geriatric medicine and PALTC regulations, along with awareness of the work habits and behaviors needed to become an integral part of the facility leadership team and a preferred clinician partner
  • Ensures clinical understanding of Value Based Practice concepts and key components of patient-centered care and clinician responsibilities in optimizing patient stays leading to good patient outcomes
  • Assists clinicians with MIPS education and documentation requirements in conjunction with the Quality Payment Program Department.
  • Assist with interviewing and hiring, troubleshooting clinician/facility issues when appropriate, and serve as a clinical liaison as needed
  • Assist clinical practice manager when clinical perspective and/or liaison needed
  • Assist with assessment, education, and action plan for provider deficiencies when identified, provide ongoing education and on-site presence to all clinicians when indicated and continually cultivate a culture of Value Based practice
  • Responsible for assisting with provider retention strategies related to their book of business
  • Performs other duties as assigned

Job Requirements

  • Significant experience as practicing APC/Physician in PALTC settings
  • Working knowledge of billing, coding and documentation requirements
  • Working knowledge and comfort with gHERiMED
  • Possess knowledge of Skilled and Long Term Care Facility structure and function
  • 2-3 years clinical and technology training experience
  • Possess excellent communication skills; oral, written and presentational
  • Possess strong leadership skills; excellent facilitator/mentor, excellent interpersonal skills and the ability to work collaboratively across departments
  • Ability to manage multiple priorities
  • Self-starter, ability to work remotely
  • Excellent time management, work flow and organizational skill
  • Out-of-state travel required. Must possess valid State Driver’s License
